Home Health Speech-Language Pathologist (SLP) Per Diem

Apollo Home Healthcare is seeking a SpeechLanguage Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa County Alameda County and the Tri-Valley area. This is a W-2 per diem role ideal for SLPs looking for schedule flexibility autonomy and meaningful patient relationships. Youll provide high-quality one-on-one care to patients in their homes while collaborating with a supportive interdisciplinary team.

As a Home Health SLP you will:

• Perform speech-language and swallowing evaluations in patients homes
• Develop and implement individualized Plans of Care
• Provide skilled speech therapy interventions
• Recommend communication devices and adaptive equipment as needed
• Assess home environments for safety and functional communication needs
• Collaborate with physicians nurses PT/OT and care teams
• Participate in case conferences as appropriate
• Complete timely compliant clinical documentation
• Educate patients and caregivers on exercises and compensatory strategies
• Supervise home health aides when applicable to the plan of care

• Active California SpeechLanguage Pathologist license
• Masters degree in Speech-Language Pathology
• Current CPR certification
• Home health experience preferred (but not required)
• Strong documentation and communication skills
• Ability to work independently in the field
• Reliable transportation and valid drivers license

• $110-150/visit $45/hour for training
