Responsibilities:
Patient Assessment: Conduct comprehensive assessments of patients physical mental and emotional health including vital signs medication administration wound care and disease management.
Care Planning: Develop individualized care plans that address patients specific needs and goals including medication administration wound care and disease management.
Patient Education: Educate patients and their families on disease management medication administration and self-care techniques to promote independence and improve health outcomes.
Clinical Interventions: Provide direct patient care including wound care medication administration injections and IV therapy.
Collaboration: Collaborate with physicians therapists and other healthcare professionals to ensure coordinated and comprehensive care.
Documentation: Maintain accurate and timely documentation of patient assessments interventions and progress notes.
Compliance: Adhere to all relevant regulatory standards including HIPAA and Medicare regulations.
Requirements:
Licensure: Current and valid Registered Nurse (RN) license in the state of Florida.
Experience: Minimum of 1 year of hospital or skilled nursing facility is required.
Skills: Strong clinical skills excellent communication and interpersonal abilities and a patient-centered approach to care.
Professionalism: Strong work ethic reliability and a commitment to providing high-quality care.
Transportation: Reliable transportation to visit patients in their homes.
