Responsibilities:
- Patient Assessment:Conduct comprehensive assessments of patients physical mental and emotional health including vital signs medication administration wound care and disease management.
- Care Planning:Develop individualized care plans that address patients specific needs and goals including medication administration wound care and disease management.
- Patient Education:Educate patients and their families on disease management medication administration and self-care techniques to promote independence and improve health outcomes.
- Clinical Interventions:Provide direct patient care including wound care medication administration injections and IV therapy.
- Collaboration:Collaborate with physicians therapists and other healthcare professionals to ensure coordinated and comprehensive care.
- Documentation:Maintain accurate and timely documentation of patient assessments interventions and progress notes.
- Compliance:Adhere to all relevant regulatory standards including HIPAA and Medicare regulations.
Requirements:
- Licensure:Current and valid Registered Nurse (RN) license in the state of Florida.
- Experience:Minimum of 1 year of experience in a hospital or skilled nursing facility
- Skills:Strong clinical skills excellent communication and interpersonal abilities and a patient-centered approach to care.
- Professionalism:Strong work ethic reliability and a commitment to providing high-quality care.
- Transportation:Reliable transportation to visit patients in their homes.
