As a Home Health Registered Nurse youll have the opportunity to build meaningful relationships with patients and families while delivering one-on-one care in the home setting. As a Home Health RN you will:
Provide skilled nursing care to patients in their homes
Perform comprehensive patient assessments and develop individualized plans of care
Monitor patient progress and adjust care plans as needed
Educate patients families and caregivers on disease management medications treatments and safety practices
Collaborate with physicians therapists social workers and other healthcare professionals to coordinate care
Complete timely and accurate clinical documentation
Identify changes in patient conditions and communicate findings to the care team
Participate in case conferences and interdisciplinary care planning
Ensure compliance with all regulatory agency and professional nursing standards
Maintain patient confidentiality and HIPAA compliance at all times
Current unrestricted Registered Nurse (RN) license in the state of practice
Minimum of one (1) year of nursing experience in a clinical care setting
Strong assessment critical thinking and communication skills
Ability to work independently and effectively manage a patient caseload
Reliable transportation a valid drivers license and current auto insurance
Willingness to travel throughout the assigned service area
Physical Requirements
Sit stand bend lift and move intermittently and be able to lift 50-100 lbs
Preferred Qualifications
Home Health experience
Hospice experience
Experience with HomeCare HomeBase (HCHB)
Associates or Bachelors Degree in Nursing
