As a Home Health Registered Nurse youll have the opportunity to build meaningful relationships with patients and families while delivering one-on-one care in the home setting. As a Home Health RN you will:

• Provide skilled nursing care to patients in their homes

• Perform comprehensive patient assessments and develop individualized plans of care

• Monitor patient progress and adjust care plans as needed

• Educate patients families and caregivers on disease management medications treatments and safety practices

• Collaborate with physicians therapists social workers and other healthcare professionals to coordinate care

• Complete timely and accurate clinical documentation

• Identify changes in patient conditions and communicate findings to the care team

• Participate in case conferences and interdisciplinary care planning

• Ensure compliance with all regulatory agency and professional nursing standards

• Maintain patient confidentiality and HIPAA compliance at all times

• Current unrestricted Registered Nurse (RN) license in the state of practice

• Minimum of one (1) year of nursing experience in a clinical care setting

• Strong assessment critical thinking and communication skills

• Ability to work independently and effectively manage a patient caseload

• Reliable transportation a valid drivers license and current auto insurance

• Willingness to travel throughout the assigned service area

• Physical Requirements

  Sit stand bend lift and move intermittently and be able to lift 50-100 lbs

Preferred Qualifications

• Home Health experience

• Hospice experience

• Experience with HomeCare HomeBase (HCHB)

• Associates or Bachelors Degree in Nursing
