Job Summary

Home Health Registered Nurse (RN) Dunedin FL

A leading healthcare organization is seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high-quality patient-centered care in the home setting. Responsibilities
  • Provide hands-on nursing care to patients in their homes using the nursing process and established clinical standards and protocols

  • Assess plan implement and evaluate individualized patient care plans

  • Oversee and coordinate the activities of the multidisciplinary team involved in patient care

  • Monitor the delivery of therapeutic services through effective written and verbal communication with all disciplines

  • Ensure all scheduled home visits are authorized and supported by physician orders

  • Conduct weekly reviews of patient visits and communicate care plans and anticipated needs to the Patient Care Supervisor and team assistant

  • Communicate schedule changes caseload updates and patient care concerns to the Home Care Supervisor

  • Maintain accurate and timely documentation of patient assessments and care provided

  • Educate patients and caregivers to promote health independence and positive outcomes

  • Collaborate with interdisciplinary team members to ensure continuity and quality of care

Qualifications
  • Graduate of an accredited School of Nursing

  • Current Registered Nurse (RN) license in the state of practice

  • Current Basic Life Support (BLS) certification

  • Valid Florida drivers license or ability to obtain one within 30 days of hire

  • Associate Degree or Diploma in Nursing required

  • Bachelor of Science in Nursing (BSN) preferred

  • Minimum of one year of Registered Nurse experience required

  • Previous home health experience preferred

  • Strong communication organizational and critical thinking skills

  • Ability to work independently and effectively manage patient caseloads

  • Must meet all applicable background screening requirements
