We are currently seeking a Full-Time Home Health Registered Nurse (RN) Case Manager to join our growing team serving Newton Needham Wayland and surrounding communities. As a Home Health RN Case Manager you will manage a caseload of patients and provide skilled nursing care in the home environment while developing your clinical expertise in a growing and dynamic setting.

• Perform comprehensive patient assessments and develop individualized care plans

• Conduct initial and ongoing skilled nursing visits based on patient needs and physician orders

• Monitor and evaluate patient status including physical psychosocial environmental and caregiver factors

• Coordinate care with physicians therapists social workers and other care team members

• Communicate changes in patient condition and recommend appropriate interventions

• Update and revise care plans as patient needs evolve

• Provide education and support to patients and caregivers

• Maintain accurate timely clinical documentation in compliance with regulations and company standards

• Ensure adherence to HIPAA and patient confidentiality requirements

• Current unrestricted Registered Nurse (RN) license in the applicable state

• Minimum of 1 year of nursing experience in a clinical setting

• Valid drivers license auto insurance and reliable transportation

• Willingness to travel within the assigned territory (approximately 50%)

• Ability to sit stand bend lift and move intermittently

• Ability to lift 50100 lbs as needed
