What Youll Do
Conduct patient admission assessments and develop care plans.
Perform supervisory visits to monitor patient care and progress.
Educate patients and families on medications conditions and care plans.
Document visits accurately and communicate changes in patient condition.
Collaborate with physicians and the healthcare team to ensure quality care.
What Were Looking For:
Licensed Registered Nurse (RN) in Georgia
Experience in home health long-term care or geriatrics is a plus
Strong verbal and written communication skills
Ability to lift up to 40 lbs using proper body mechanics
Compassionate reliable and self-motivatedwith a cool head in tough situations
