How will you make an impact & Requirements

Responsibilities:
Patient Assessment: Conduct comprehensive evaluations of patients physical abilities limitations and functional needs to accurately diagnose impairments and develop individualized treatment plans.
Therapeutic Interventions: Implement a variety of therapeutic techniques including manual therapy therapeutic exercises and gait training to improve patients mobility strength balance and pain management.
Functional Training: Assist patients in developing compensatory strategies adaptive techniques and assistive device training to enhance their independence in activities of daily living.
Patient Education: Educate patients and their families on proper body mechanics home exercise programs and injury prevention strategies to promote long-term health and well-being.
Collaboration: Work collaboratively with physicians nurses occupational therapists and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
Documentation: Maintain accurate and timely documentation of patient evaluations treatment plans progress notes and discharge summaries.
Compliance: Adhere to all relevant regulatory standards and guidelines including HIPAA and Medicare regulations.

Requirements:
Licensure: Current and valid Physical Therapist license in the state of Florida.
Experience: Minimum 6 months of experience in a hospital skilled nursing facility or rehab is requested.
Skills: Strong clinical skills excellent communication and interpersonal abilities and a patient-centered approach to care.
Professionalism: Strong work ethic reliability and a commitment to providing high-quality care.
Transportation: Reliable transportation to visit patients in their homes.
