Elara Caring is seeking a compassionate and skilled Registered Nurse to join our growing Home Health team on a PRN this role youll enjoy the independence of managing your own schedule while providing one-on-one care to patients in the comfort of their homes.

As a Home Health Registered Nurse you will manage a caseload of patients and provide comprehensive nursing care throughout their healthcare journey.

Responsibilities include:

• Completing skilled nursing assessments and developing individualized plans of care

• Performing admissions routine visits recertifications and discharge visits

• Evaluating patients physical psychosocial environmental and educational needs

• Coordinating care with physicians caregivers and interdisciplinary team members

• Monitoring patient progress and identifying changes in condition

• Updating and revising care plans to ensure optimal outcomes

• Educating patients and families on disease management medications and self-care techniques

• Maintaining accurate and timely clinical documentation

• Helping prevent avoidable hospitalizations through proactive intervention and patient education

• Delivering compassionate patient-centered care while promoting safety and independence

• Current unrestricted Massachusetts Registered Nurse (RN) license

• Minimum of one (1) year of nursing experience in a clinical care setting

• Home Care experience is HIGHLY preferred as this is PRN and having knowledge of how home care operates will help to make you successful

• Ability to commit to at least one full day (8 hours) per week

• Valid drivers license reliable transportation and current auto insurance

• Willingness to travel throughout the assigned territory

• Physical Requirements - Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
