PT: $80 per visit
PTA: $55$60 per visit
Location: Oahu HI (reliable transportation required)

We are currently hiring licensed Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) who are passionate about providing high-quality patient-centered home health care.

skilled in-home physical therapy services
Complete evaluations reassessments and fall risk assessments (PT)
Implement individualized treatment plans to improve mobility balance strength and safety
Educate patients and caregivers on home exercise programs and fall prevention
Document accurately and communicate patient progress with the interdisciplinary care team
Maintain compliance with Medicare home health documentation requirements
Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
Minimum one year of clinical experience (home health experience preferred)
Strong documentation communication and time-management skills
Ability to work independently in the field
Valid drivers license and reliable transportation
Passion for serving seniors kupuna and local families
