Compensation: $80 per visit

We are seeking a licensed Physical Therapist (PT) who is passionate about delivering high-quality patient-centered care directly to local families and kupuna.

Conducting comprehensive initial evaluations ongoing reassessments and detailed fall risk assessments.
Designing and executing individualized treatment plans focused on balance strength and spatial safety.
Empowering patients and their multi-generational caregivers through tailored home exercise program education.
Ensuring accurate compliant clinical documentation and collaborating smoothly with our interdisciplinary care team.
What We Are Looking For:
Active Hawaii license as a Physical Therapist (PT).
Minimum of one year of clinical experience (home health experience is a huge plus but we welcome strong clinical backgrounds).
Excellent communication time management and independent clinical documentation skills.
Valid drivers license and reliable transportation to travel to patient homes.
A genuine dedication to serving seniors and giving back to our local Oʻahu community.
