We are currently seeking licensed Physical Therapists to provide home health services across Oʻahu.

Responsibilities

• Provide skilled physical therapy visits in patients homes
• Conduct PT evaluations and follow-up assessments
• Develop individualized treatment plans focused on mobility strength balance and fall prevention
• Educate patients and family caregivers on safe mobility techniques
• Maintain accurate Medicare-compliant documentation
• Collaborate with the care team to support patient progress

• Current Hawaii Physical Therapist (PT) license
• Minimum 1 year clinical experience (home health experience preferred)
• Strong documentation and communication skills
• Ability to work independently in a home setting
• Valid drivers license and reliable transportation
