- Lead and manage home health operations
- Supervise nursing PT and OT services in home settings
- Hire train coach and develop home care staff
- Ensure care plan execution and documentation accuracy
- Monitor and improve quality of patient care
- Oversee care delivery in:
- Patients homes
- Nursing homes
- Board & care facilities
- Drive workflow compliance and care standards
- Participate in interdisciplinary team (IDT) coordination
- RN License (California)
- Minimum 2 years nursing experience in:
- Home health
- OR elderly/frail patient care
- At least 1 year leadership experience (preferred)
- Bachelors Degree (or equivalent experience)
- Strong home health background (must-have focus)
- Experience managing field-based care teams
- Familiar with elderly/frail patient population
- Proven leadership in:
- Staff oversight
- Care coordination
- Quality improvement
- Comfortable in value-based care environments
