As a Home Health LPN you will provide skilled nursing care under the direction of a Registered Nurse and physician while supporting patients and families throughout their healthcare journey.
* Delivering care in accordance with physician orders delegated nursing responsibilities and the established Plan of Care
* Monitoring patient conditions and reporting significant changes to the RN Case Manager and healthcare team
* Assisting patients and caregivers with education and self-care techniques
* Implementing patient safety plans and utilizing available community resources
* Collaborating with the interdisciplinary care team to ensure exceptional patient outcomes
* Documenting clinical findings nursing interventions and medication updates accurately and timely
* Identifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits
* Providing compassionate professional support to patients families physicians and colleagues
* Graduate of an accredited Certificate Diploma or Associate Degree Nursing program
* Current Massachusetts Licensed Practical Nurse (LPN) license
* Minimum of one (1) year of clinical nursing experience
* Valid drivers license reliable transportation and current auto insurance
* Ability and willingness to travel throughout the assigned territory
* Physical Requirements - Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.
