Location: Roanoke VA (School-Based One-on-One Patient Care)

Schedule: MondayFriday 9:00 AM 3:00 PM Year-Round Position (Student attends school during summer months)

Start Date: July 7 2026

Pay: $30/hour (based on experience)

We are currently seeking a compassionate and skilled Home Health Licensed Practical Nurse LPN to provide one-on-one nursing care for an elementary school-aged patient in a school setting in Roanoke VA.

This position is ideal for an LPN who enjoys building meaningful relationships with pediatric patients while providing skilled nursing care in a supportive educational environment. The nurse will accompany and care for the student during the school day ensuring the patients health safety and medical needs are met in accordance with the physicians orders and plan of care.

• Provide one-on-one skilled nursing care during the students school day

• Monitor and assess the patients condition and report any changes as appropriate

• Perform routine gastrostomy tube (G-tube/PEG tube) site care and maintenance

• Administer enteral feedings via feeding tube as ordered by the physician when oral intake is insufficient

• Maintain accurate and timely documentation of care provided

• Collaborate with parents school personnel and the healthcare team to ensure continuity of care

• Promote a safe and supportive environment for the patient throughout the school day

• Respond appropriately to changes in patient condition and follow established care plans

• Active LPN license in the state of Virginia

• Pediatric nursing experience preferred but not required

• Experience with feeding tubes enteral nutrition or gastrotube care preferred

• Strong assessment clinical and critical-thinking skills

• Compassionate dependable and patient-focused

• Ability to work independently in a school-based setting

• Current CPR certification
