Full-Time Monday through Friday 8a to 5p

What Youll Do

As a Home Health LPN youll provide skilled nursing visits in patients homes. Working under the supervision of a Registered Nurse you will:

  • Deliver high-quality skilled nursing care according to physician orders and individualized care plans

  • Monitor patient conditions and identify changes requiring intervention

  • Provide wound care medication management disease education and other skilled nursing services

  • Educate patients and caregivers on self-care techniques treatment plans and safety measures

  • Collaborate closely with RN Case Managers physicians therapists and other members of the care team

  • Document patient visits accurately and timely in the electronic medical record

  • Assist in implementing care plans designed to improve outcomes and reduce avoidable hospitalizations

  • Communicate significant findings and patient concerns to the interdisciplinary team

  • Promote patient independence safety and quality of life in the home setting

Schedule & Coverage

  • Full-time position

  • Flexible scheduling with support from local clinical leadership

What Youll Need
  • Graduate of an accredited Practical Nursing program

  • Current unrestricted Licensed Practical Nurse (LPN) license

  • Minimum of one (1) year of nursing experience in a clinical setting

  • Valid drivers license reliable transportation and current auto insurance

  • Ability to travel throughout the assigned service area

  • Strong communication assessment and organizational skills

  • Physical Requirements: Sit stand bend lift and move intermittently and be able to lift 50-100 lbs.

Preferred Qualifications

  • Previous Home Health experience

  • Experience working with adult and geriatric patient populations

  • Experience with electronic medical record systems
