Pod 1: Waukee Grimes Johnston areas

Shift: Monday - Friday 8:00 AM - 5:00 PM( monthly on-call)

No holidays or weekends!

The Home Health Clinical Preceptor - Nursing plays a vital role in shaping confident competent clinicians by leading clinical orientation validating competencies for both new and current employees and supporting student observation experiences. This role blends hands-on patient care with education coaching and mentorship using best practices and adult learning principles to support continuous growth. Clinical Preceptors serve as trusted guides and role models while also managing a diverse community-based caseload and delivering high-quality patient-centered care.

Responsibilities

Facilitates coordinated transitions from orientation program to independence within targeted goal.

Tracks required orientation progress and provides progress to the Clinical Supervisor

Demonstrates competency and proficiency in case management for provision of care clinical expertise documentation utilization effectiveness patient experience and quality outcomes.

Completes all documentation accurately and submits to office within 24 hours of visit phone call and/or change in treatment or orders.

Makes recommendations related to training and skills development to clinical education team.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses

Licensure/Certiciations:

  • Valid drivers license when driving any vehicle for work-related reasons.
  • Auto insurance
  • Current license to practice nursing according to state where service is provided
  • COS-C certification must be obtained within 12 months of hire.
  • Valid Mandatory Reporter course completion by state requirement.
  • BLS

Minimum Experience:

  • 1 year of home health or hospice case management

Other:

  • Use of usual and customary equipment used to perform essential functions of the position.
