The first home health visit sets the tone for everything that follows. At Elara Caring were looking for an experienced RN who can walk into a patients home see the full picture and build a thoughtful plan for what comes next.

As an Admissions RN youll focus on start-of-care visits across Greater Boston and the North Shore. Youll have the independence to use your clinical judgment with a team behind you to help patients move smoothly into ongoing care.

• Complete start-of-care visits and comprehensive assessments
• Evaluate clinical needs medications home safety and caregiver support
• Create individualized plans of care with physicians and the clinical team
• Educate patients and families so they understand the plan and feel prepared
• Document admissions accurately and communicate key findings to the care team

• An active unrestricted Massachusetts RN license
• At least one year of nursing experience
• Home health experience is required; admissions or start-of-care experience is preferred
• Strong assessment documentation and independent decision-making skills
• A valid drivers license reliable transportation and current auto insurance

Experience with HomeCare HomeBase (HCHB) is a plus. An associate degree in nursing is required; a BSN is preferred.
