Schedule: Full-time 40 hours per week 8am - 4:30pm Monday - Friday
Department: Home Care PACE of Southwest Michigan Van Buren County
Travel Requirements: This role involves regular travel providing Home-Based Care. You must live in Van Buren County for this position to be a feasible commute. You will be assigned participants living in Van Buren County.
Check in with the PACE Day Center in St. Joseph once a week as needed.

You will work closely with the Home Care Supervisor to deliver compassionate person-centered care and support activities of daily living. From assisting with daily activities to providing comfort and companionship your work will make a meaningful impact on the lives of those we serve.

Key Responsibilities
- Provide Hands-On Care in Their Home Assist with personal care (ADLS) like bathing grooming dressing and toileting.
- Support Mealtime Help participants enjoy their meals and stay hydrated for good health.
- Keep Things Moving Support mobility including turning positioning and safe transfers.
- Perform Vital Checks Measure and record vital signs weight and intake/output.
- Maintain Comfort & Care Support housekeeping tasks such as laundry sweeping and grocery shopping.
- Provide Responsive Support Answer call lights quickly and respond with kindness and compassion.
- Monitor with Watchful Eyes Report any changes in conditions to the leadership ASAP.
- Demonstrate Respect & Professionalism Maintain confidentiality while delivering top-notch care.

Skills Knowledge and Expertise:
- One (1) year of prior experience with frail or elderly individuals aiding with daily living activities.
- Valid Michigan Drivers License with a clean driving record and reliable transportation.
- CNA certification desired.
- Completion of the Participant Care Assistant (PCA) program at PACE of Southwest Michigan or an equivalent training program if not a Certified Nurse Assistant.
- Current CPR or BLS certification or willing to obtain.
- Ability to work independently multitask and operate as part of an interdisciplinary team.
