Join a collaborative interdisciplinary team dedicated to CentraCares mission of enhancing every life we touch with care and this specialized role youll make a meaningful impact on the lives of patients living with heart failure by providing education support and ongoing disease management across the continuum of care. Your responsibilities will include bedside care in the hospital setting managing new and follow-up visits in the outpatient clinic and conducting occasional remote work from home. This position requires a strong commitment to the heart failure population excellent clinical judgment and the ability to work independently with a high level of self-motivation and dedication to patient outcomes.
Schedule:
- Full-time 60 hours every two weeks St. Cloud Hospital
- Day 7am-4:30pm Monday-Friday
Pay and Benefits:
- Pay based on experience and degree
- Associate/Diploma in nursing starting at $42.86 per hour
- Bachelors in nursing starting at $44.34 per hour
- Masters in nursing starting at $45.82 per hour
- Pay range: $42.86- $71.51 per hour

Qualifications:
- Graduate from an accredited nursing program required
- Current Minnesota RN Licenserequired
- BSN or MSN preferred
- BSN or higher required within 36 months of being hired
- Basic Life Support (BLS) Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy.
- National Certification in Advanced Heart failure within 24 months preferred.
- 1 year of recent cardiology work (i.e. critical care nursing heart catheterization emergency room) preferred.
- 2 years of experience working in a cardiovascular disease program preferred
- Good written and verbal communication skills.
- Sufficient knowledge to recognize abnormal laboratory data and other diagnostic testing results n
