Procedures/Duties:
- Routine medical evals and chronic disease management
- Annual wellness and preventive care assessments
- Management of dementia frailty and other complex chronic conditions
- Oversight of palliative and comfort-focused care plans when appropriate
- Review and approve admissions to SNF incl completing comprehensive H&P exams.
- Conduct physician compliance and recertification visits
- Provide medical care for long-term care residents
- Develop review and update individualized care plans
- Manage acute changes in condition evaluate new symptoms
- Prescribe monitor and adjust medications while reviewing consultant pharmacist recommendations and ensuring medication safety.
- Participate in interdisciplinary care conferences family meetings and goals-of-care discussions as appropriate
- Complete required documentation physician orders certifications
