Full Time Occupational Therapist

• Administers and interprets appropriate occupational therapy evaluation procedures and best practice outcome measures for individual patient need
• Appropriately analyzes evaluation information to formulate an individualized occupational therapy plan of care including developing a occupational therapy assessment problem list interventions recommended frequency and duration of OT services pertinent goals and discharge planning to maximize a patients mobility and function
• Ensures goals are specific measurable attainable relevant and time-bound for individual patient need
• Plans and delivers evidence-based intervention based on evaluation and plan of care
• Exercises sound clinical decision making and maintains patient safety when planning and evaluating the patients response to treatment and evaluation.
• Performs patient family and/or care-partner education and training on OT diagnoses prognosis and ADL/self-care and other needs including performance of home evaluations

• Supervise and direct rehab tech as needed

• Consult with and refer to external medical practitioners when needed in collaboration with the medical director
• Uses evaluation and treatment findings to determine appropriate discharge planning needs.
• Evaluates need for adaptive and/or durable medical equipment/bracing/splinting and completes justification as necessary for ordering in a timely manner and in collaboration with
• Completes all required occupational therapy documentation and billing in accordance with the policies and procedures
• Supervises certified occupational therapy assistants (COTAs) and rehab techs providing consistent communication and consultation about the OT plan of care and subsequent changes
• Assists with staff education training and competencies as appropriate within licensure
• Participates in individualized patient plan meeting and team rounds as warranted by patient need and determined by treatment team
• Assists with staff education training and competencies as appropriate within licensure
• Actively participates in intra-departmental staff meetings and intra-discipline/department committee assignments.
• Participates as a positive and collaborative member of the interdisciplinary team and inter-program team to ensure patient-centered care
• Adheres to the professional behaviors expectations Code of Conduct Code of Ethics all Policies and Procedures and rules and ethics of licensure.
• Responsible for the reporting of employee/patient/visitor injuries or accidents concerns about abuse or neglect or other safety issues to the supervisor and in the occurrence notification system.
• Actively participates in center-wideinitiativesand staff training that affect occupational therapy practiceand scope
• Willparticipate in education programs by teachingand/orsupervising students based on clinician experience and organizational need.
• Participates in research projectsaswarranted by departmental and/or organizational needs.
• Performs other duties as required or requested by supervisor
