What You Will Do

  • Float between Skilled Nursing and Assisted Living neighborhoods based on resident care and staffing needs
  • Administer medications and treatments in accordance with physician orders and facility policies
  • Monitor residents conditions and communicate changes to physicians supervisors and care team members
  • Assist with resident assessments care planning and MDS-related documentation as assigned
  • Perform direct resident care including activities of daily living (ADLs) such as bathing dressing grooming toileting mobility assistance and feeding assistance
  • Provide restorative nursing support to help residents maintain or improve functional abilities
  • Assist with admissions transfers and discharges
  • Complete accurate and timely documentation in compliance with regulatory requirements
  • Participate in audits chart reviews infection prevention activities and quality assurance initiatives
  • Support residents and families through compassionate communication and resident-centered care
  • Respond appropriately to emergencies and changing resident care needs

What You Bring

  • Current Licensed Practical Nurse (LPN) license in the Commonwealth of Virginia
  • Graduation from an approved Licensed Practical Nursing program
  • Previous experience in long-term care skilled nursing assisted living or restorative nursing preferred
  • Excellent communication organizational and time management skills
  • Strong critical thinking and problem-solving abilities
  • Ability to work independently and collaboratively within an interdisciplinary team
  • Flexibility to work across multiple care settings and adapt to changing assignments
  • Commitment to providing exceptional resident-centered care
