This role focuses on strengthening family protective factors enhancing caregiver capacities promoting child safety and supporting family stability through culturally responsive trauma-informed and evidence-based interventions and support :Work directly with families involved with the Department of Child Safety to support child safety and family an assigned caseload and ensure timely completion of required services documentation referrals and follow-up home visits and community-based meetings to assess family needs monitor progress and provide supportive assessments to help ensure a safe environment for trauma-informed family-centered interventions and support related to parenting mental health domestic violence substance use and other identified family with families to provide consultation education and skill development related to family functioning and services that strengthen family protective factors and enhance caregiver with service planning linkages and transition to ongoing supports including helping families access financial social medical educational and employment substance use education and linkage to treatment providers as observe and document interactions and behaviors between parents/caregivers and children during home visits including modeling appropriate behavior and de-escalating cooperatively with the Department of Child Safety (DCS) to support assessments service planning required reports and caring supportive strength-based culturally competent and trauma-informed relationships with families and community accurate timely and thorough documentation including assessments progress notes service plans incident reports activity logs statistical reports and other required records in accordance with program contractual funding and regulatory and maintain confidential client records and case record and submit time worked in accordance with organizational policies and wage and hour in an on-call rotation and respond to client or program needs in accordance with program professional representation of A New Leaf in all interactions with clients staff and community meetings trainings and special projects as requested by other reasonably related duties as :Education:Bachelor of Social Work or a related bachelors degree :Minimum of one (1) year of experience in the human services working with multi-problem families and complex family of family dynamics and family-centered service of community resources and ability to assist families in accessing supporting families impacted by substance abuse child abuse and neglect trauma sexual abuse or related working with families involved in child welfare family preservation family reunification or Department of Child Safety programs Conditions/Physical Requirements:Work is performed in office settings community locations client homes and other environments where services are local travel is required to conduct home visits attend meetings and provide community-based be exposed to a variety of environmental conditions and unpredictable situations while working in client homes and community use of computers telephones and standard office to sit stand walk bend reach and drive for extended periods throughout the lifting and carrying of up to 25 to manage multiple priorities and deadlines in a fast-paced be required to work occasional evenings weekends or modified schedules to meet client and program accommodations may be made to enable individuals with disabilities to perform the essential functions of the Requirements: Must Be At Least 21 Years of AgeClass One Fingerprint Clearance CardCentral Registry Background CheckDrug ScreeningCPR/First Aid CertificationValid Unrestricted Drivers LicenseCurrent Auto Liability Insurance39 Month Motor Vehicle RecordUse of a personal vehicle to conduct agency business and transport clients when to obtain and maintain all required certifications clearances licenses and insurance throughout to all organizational policies including the Code of Conduct professional standards and relevant Reporter Requirements:As a mandated reporter under Arizona statute A.R.S. 13-3620 you are required to immediately report any suspected abuse or neglect of a minor to your Supervisor Manager Director Clinical Director or Chief Officer who will assist in making the appropriate report to should be directed to either the local law enforcement agency or child protective services (1-888-SOS-CHILD).
