Role Scope: The Enrollment RN plays a key role in supporting Habitat Healths growth by serving as the clinical lead throughout the enrollment partnership with the Outreach Specialist Enrollment Specialist and the Interdisciplinary Team (IDT) this role conducts clinical eligibility and home safety assessments provides education to prospective participants and caregivers and supports informed enrollment decisions. The Enrollment RN helps ensure a seamless transition into the PACE program while delivering a welcoming participant-centered experience and maintaining compliance with applicable regulatory requirements.
Core Responsibilities & Expectations for the Role
- Partners closely with the Outreach Specialist Enrollment Specialist and Interdisciplinary Team (IDT) to support a coordinated participant-centered enrollment experience.
- Conducts comprehensive nursing home safety and eligibility assessments in participants homes and other care environments to support enrollment decisions.
- Completes Nursing Facility Level of Care (NFLOC) assessments and related documentation in accordance with DHCS requirements ensuring timely submission and follow-up as needed.
- Participates in enrollment eligibility meetings by presenting clinical findings and collaborating with the IDT to support informed enrollment recommendations.
- Conduct home safety evaluations of the prospective participants home
- Builds trusted relationships with prospective participants and caregivers by providing education addressing clinical questions and delivering a welcoming and responsive enrollment experience.
- Maintains accurate clinical documentation and participant records within electronic health record (EHR) and related documentation systems in a timely and accurate manner.
- Maintains knowledge of PACE eligibility requirements enrollment processes and applicable federal and state regulatory requirements.
- Supports organizational growth by participating in outreach activities and community engagement initiatives as needed.
- Monitors assigned enrollment activities and contributes to continuous improvement by identifying opportunities to enhance participant experience workflows and operational effectiveness.
- Performs other related duties as assigned.
Required Qualifications:
- Associates degree in Nursing from an accredited nursing program or equivalent nursing education required.
- Active unrestricted Registered Nurse (RN) license in the applicable state.
- Minimum of one (1) year of nursing experience working with older adults frail elderly populations or individuals with complex medical needs.
- Demonstrated ability to conduct comprehensive clinical and home safety assessments while applying sound clinical judgment to support participant eligibility and enrollment decisions.
- Strong organizational skills with the ability to manage multiple priorities while maintaining attention to detail and ensuring timely follow-through.
- Strong communication and collaboration skills with the ability to effectively engage prospective participants caregivers providers and cross-functional teams to support a seamless enrollment experience.
- Demonstrated ability to work independently while collaborating across interdisciplinary teams to achieve shared outcomes.
- Experience using electronic health records (EHRs) and the ability to quickly learn enrollment management and clinical documentation systems.
- Proof of a valid drivers license reliable personal transportation and automobile insurance that meets applicable state requirements with the ability to travel throughout the service area.
- Active CPR/BLS certification or the ability to obtain certification within 30 days of hire.
- Meets all applicable participant-facing health requirements including TB screening and required immunizations.

Nice to have:
- Bachelors degree in Nursing (BSN).
- Experience working within PACE Medicare Medicaid managed care or other value-based healthcare environments.
- Knowledge of PACE CMS and applicable federal and state regulatory requirements related to participant eligibility enrollment and care coordination.
- Experience conducting Nursing Facility Level of Care (NFLOC) home safety or other clinical eligibility assessments.
- Bilingual: Spanish Mandarin or Cantonese.

Compensation:
The anticipated base salary range for this position is $50.00 - $61.00 hourly. This position is also eligible for a target annual bonusand may be eligible for additional incentive compensation as applicable.
The final compensation offered will be determined based on factors including but not limited to the candidates qualifications skills experience education certifications geographical location and business needs.
