Job Summary

Empanelment Analyst
The Empanelment Analyst supports the organizations provider empanelment program bymaintainingaccuratepatient-provider assignmentsmonitoringpanel performance and developing reports and dashboards to support operational decision-making. This position analyzes panel data supports continuity of care initiativesassistswith provider capacity management and collaborates with clinical and operational teams to ensure the accuracy and integrity of empanelment data.

This role is located in Midtown - Sacramento CA (95811).
This role allows a hybrid schedule requiring 3 days per week on-site.
ESSENTIAL FUNCTIONS
  • GeneratemonthlypanelreportsandreviewPCPassignments
  • Calculatecontinuityofcarepercentagesforeachprovider;adjustforvacationcoverageschedulesandotherfactors
  • Resolveunassignedpatientsbyreviewingappointmenthistory(andpossiblytheclinicalrecord)todetermineappropriate assignment
  • Monitorinfluxofnewpatientsintoproviderpanelsandanalyzeprovidercapacitytoabsorbnewpatients;review issues withVP of Health Services andChiefHealthOfficer and reassign asappropriate.
  • Assesspanelsizeagainstrightsizeanddiscussanyneededadjustmentswiththe VP of Health Services andtheChiefHealthOfficerand/orthemedicalmanagement team
  • Trackpatientstatuschangesincludingdeathtransfertoothercareandmakeneededchangesinthepractice management system
  • Reviewtransferofcarerequestsincluding:
  • Patient-initiatedPCPchangerequests;facilitatediscussionbetweenandobtainapprovalfromcurrentassigned PCP and future PCP; make the change in the practice management system
  • ; obtain agreement of acceptance by the new PCP; make the change in the practice management system
  • Trackproviderstatuschanges(transferterminationsabbaticalmedicalleaveetc.)and:
  • Evaluateimpactonassignedpanelofpatients
  • Reassignpaneltootherclinicprovidersaccordingtopanelcapacity
  • NotifyaffectedpatientsofnewPCPassignment
  • Collaboratewithclinicmanagersprovidersandcareteamsintheimplementationandongoingmaintenanceofthe provider empanelment process
  • Assistinthedevelopmentofworkflowsandprotocolstomaximizeoutcomereporting
  • Owns the panel reporting process including data cleanup provider alignment and validation to ensure accuracy and integrity of empanelment data
  • Develops dashboards and reporting tools to support panel management empanelment and risk stratificationutilizingsystems such as Azara EPIC OCHIN and BI reporting tools
  • Partners with IT and Population Health teams tooptimizedata sources and reporting workflows that support patient outreach recall initiatives and care gap closure
  • Developandimplementdatastrategiesthatoptimizeefficiencyanddataquality
  • AssistwithdailyreviewappointmenttoensurepatientsarescheduledwithPCP
  • Adheretoattendanceandpunctualitypolicies
  • Maintainestablisheddepartmentalpoliciesandprocedures
  • Supportthevisionmissionandgoalsanddemonstrationcommitmentthevaluesoftheorganization
  • Enhanceprofessionalgrowthanddevelopmentthroughparticipationineducationalprogramscurrentliteraturein- service meetings and workshops

MINIMUM REQUIREMENTS
  • Bachelors degree in healthcare business public health analytics or a related field; or equivalent combination of education and experience.
  • Two years of experience in healthcare operations healthcare analytics panel management population health or related field.
  • Intermediateadvancedproficiencyin Microsoft Excel.
  • Experience working with healthcare data reports and operational metrics.
  • Strong analytical organizational and problem-solving skills.
  • Ability to interpret data and communicate findings to operational and clinical stakeholders.
  • Experience with electronic health records and healthcare reporting systems
