- POSITION INFORMATION
- Position summary:
The Emergency Department Social Worker provides psychosocial assessment discharge planning care coordination and resource management service for patients presenting to the Emergency Department. The social worker collaborates with physicians nurses case managers community agencies patients and families to facilitate safe and timely transitions of care reduce avoidable readmission and address social barriers that impact health outcomes.
- MINIMUM QUALIFICATIONS
- Education:
- Bachelors degree required. Masters Level Degree in Social Work preferred.
Required length and type of experience:- Two years work experience is required in a hospital emergency department case management or medical-social work.
- Knowledge of post-acute care services community resources Medicare Medicaid and discharge planning processes preferred.
Required licensure certification or registry:- Licensed Social Worker in the state of Ohio.
- LISW or LISW-S preferred.
