Elder Care Transition Specialist
The Elder Care Transition Specialist will provide direct assessment of individuals enrolled in ACCS as well as consultation and support to the team. The Elder Care Specialist will assist clinicians in assessment identifying appropriate level of care (LOC) developing plans for support and transition to nursing and rehab facilities as well as senior living or rest home.
- Conduct assessments in collaboration with team Nurse Occupational Therapist Clinician as needed and assist in establishing treatment plans.
- Consult with the team on necessary elements to successful aging in place
- Build collaborative relationships with area nursing facilities rest homes senior services etc.
- Provide consultation training education and resources to families treatment team and ACCS staff.
- Establish and maintain collaborative relationship with DMH Nurse Manager
- Develop a through understating of PASSR regulations and requirements
Schedule: Full Time 40 hours
Pay Rate: $25.00/hourly

- Must have a valid drivers license and regular access to a reliable vehicle.
- Must be able to work as a member of a team and to function independently.
- Bachelors Degree preferred
- At least three years working within elder care either in the community or in a facility required.
- Degree in one of the following preferred: nursing occupational therapy social work
