Position Purpose

In the Emergency Department (ED) at Saint Francis Hospital and Medical Center the Nurse Senior Navigator informally known as the Complex Care Coordinator plays a crucial role in supporting emergent and high-risk patients with complex medical social and/or behavioral health needs. The Registered Nurse (RN) in this role collaborates closely with the ED care team to deliver coordinated patient-centered care and support safe effective transitions across the continuum.

What You Will Do

• Conduct comprehensive real-time assessments of patients medical psychosocial and behavioral health needs upon presentation to the Emergency Department.

• Collaborate closely with Emergency Department physicians advanced practice providers nursing staff case management and ancillary teams to support individualized care planning for complex and high-risk patients.

• Coordinate referrals and facilitate connections to appropriate inpatient services specialty providers community resources and support programs to promote safe dispositions and effective transitions of care.

Minimum Qualifications

Required:

• Graduation from an accredited nursing program with a Bachelor of Science in Nursing (BSN) strongly preferred and generally expected for successful candidates.

• Valid RN license in Connecticut.

• The ideal candidate will have a minimum of three (3) years of Emergency Department and/or acute inpatient nursing experience with demonstrated ability to manage complex high-risk patient populations.

Preferred:

• Emergency Department Case Management or care coordination experience strongly preferred.

• BLS/CPR certification (American Heart Association or American Red Cross).
