District Homebound Teacher

TITLE: Homebound Teacher

QUALIFICATIONS: Valid Kentucky Certification

REPORTS TO: Director of Pupil Personnel and Superintendent

PERFORMANCE STANDARDS:

1. Keeps the Director of Pupil Personnel and Superintendent fully informed of the programs activities and potential problems.

2. Serves on the district Home/Hospital committee to determine if a student meets the criteria to be eligible for Home/Hospital services.

3. Meets with each assigned student according to the state Home/Hospital guidelines for the purpose of providing instruction.

4. Implements the regular classroom teachers instructional program.

5. Maintains regular contact with students teachers and other staff as necessary.

6. Administer tests and return to the regular classroom teacher.

7. Designs instruction and assessment for individual students when appropriate.

8. Encourages students to complete their assignments in a timely manner.

9. Maintains necessary records for state and local reports.

10. Makes arrangements for a substitute when absent and inform the DPP.

11. Maintains an orderly system of record keeping (including the form which show the dates and times visits were made to students) and travel records.

12. Maintains prompt and regular attendance.

13. Performs other duties as may be assigned by the DPP or Superintendent.
