Director of Case Management Opportunity:

• Full-time permanent position
  
• Director of Case Management manage the daily operations and HR functions of the Case Management department
  
• Directs the interdisciplinary care discharge planning and patient outcomes while championing patient advocacy
  
• Guides and trains staff builds relationships with healthcare and insurance networks and fosters a positive celebratory work culture
  

• Current CCM or ACM certification or eligibility to obtain within first year of employment
  
• Must possess a bachelors degree; graduate degree is preferred
  
• Three years of hospital-based Case Management experience including Utilization Review and Discharge Planning experience
