Responsibilities
- Provideperson-centered supportin aprivate homeand the community
- Implement Individual Support Plans behavior supports and therapeutic routines
- Assist withlife skills household responsibilities and community activities
- Coordinate medical appointments and support overall health and wellness
- Collaborate with families caregivers clinicians and team members
- Document services and maintain confidential records in accordance with agency standards
Qualifications
Education and Experience:
- High school diploma or equivalent required.
- Experience in direct support disability services or related areas.
License/Certification:
- Drivers License (Required)
