Pay: $22.50hr

Responsibilities
  • Direct Support: Provide hands-on assistance with personal care daily living activities and goal achievement for individuals served.
  • Case Management: Develop and maintain Individual Support Plans (ISPs) ensuring timely follow-up and delivery of support services.
  • Team Leadership: Supervise and mentor staff evaluate performance and provide contextual training in therapeutic models.
  • Behavioral Support: Design and implement behavior management interventions in collaboration with the clinical team.
  • Program Oversight: Act as the sites contact person in the absence of management ensuring smooth operations and program integrity.
  • Community Engagement: Collaborate with families guardians and external agencies to promote individual progress in home work and community settings.
Qualifications
Education & Experience:
  • Bachelors degree preferred.
  • 1-3 years of related experience including staff supervision and case management.
  • Valid standard drivers license (Class C or D).
Skills & Knowledge:
  • Expertise in activity planning behavior management and therapeutic techniques.
  • Strong understanding of person-centered planning and daily living assistance.
  • Ability to foster independence choice and growth in a respectful and culturally appropriate manner.
  • Familiarity with Seven Hills Foundation policies and procedures and program-specific regulations.
