As a direct support professional you will provide essential care and support to individuals with a diverse spectrum of abilities and challenges. This may include assisting with daily living activities promoting social and communication skills and supporting community engagement. This role will work closely with a multidisciplinary team to implement individualized plans that align with each persons goals and needs while maintaining quality service excellence and ethical standards.
Pay: $24.50/hr

Provide respectful person-centered support that fosters independence and honors each individuals unique strengths and preferences
Deliver individualized care and assistance in the home and community including implementing daily routines personal care and therapy programs
Support skill-building in daily living community participation and social engagement to promote valued roles and inclusion
Collaborate with clinicians medical providers family members and support teams to ensure holistic and consistent care
Promote and maintain existing relationships while helping individuals build new connections and community membership
Accompany individuals to appointments and community outings ensuring health safety and well-being
Implement ISP/Support Plans as written and document progress through accurate data collection and recordkeeping
Manage personal funds and assist with shopping budgeting and maintaining receipts in accordance with agency guidelines
Safeguard confidential information and comply with all HIPAA and agency privacy policies
Maintain communication and strong relationships with family members guardians and support staff
Participate in regular supervision team meetings and professional development training
Foster a cooperative respectful team environment through effective communication and shared responsibility
Provide on-call coverage approximately once per month and respond to after-hours needs as required
Follow all organizational policies and assist with basic facility maintenance including snow and ice removal during winter months
Perform other duties as assigned by program leadership
- High school diploma/GED/HiSET required
- 0 3 years of related experience
- Standard drivers license class C or class D
- CPR/First Aid required
