Job Summary
Positions Locations:
Cheshire CT 6am-4pm / 2pm-10 pm
Prospect CT 6am-4pm / 2pm-10 pm
North Haven CT 2 pm - 10 pm
Essential Responsibilities
- Provide direct care and support with activities of daily living including personal care hygiene dressing and meal preparation.
- Administer medications as trained and authorized while accurately documenting medication administration and observations.
- Assist individuals in developing daily living social and independent living skills.
- Encourage personal choice self-advocacy and participation in community activities.
- Support recreational educational and vocational activities both in the home and throughout the community.
- Maintain accurate documentation including daily notes incident reports medication records and required program documentation.
- Assist with household responsibilities such as cleaning laundry organizing and maintaining a safe living environment.
- Support individuals with budgeting shopping and other personal financial activities as appropriate.
- Provide safe transportation to appointments community outings and activities using agency vehicles.
- Monitor individuals health behavior and overall well-being reporting concerns promptly to supervisors.
- Respond appropriately to emergencies and follow established safety procedures.
- Participate in required training staff meetings and ongoing professional development.
- Communicate effectively with individuals served family members coworkers healthcare providers and supervisors.
- Follow agency policies state regulations and best practices for quality care.
- Perform other related duties as assigned.
Qualifications
- High school diploma or GED required.
- Previous experience in direct care caregiving healthcare or human services is preferred but not required.
- Valid drivers license with an acceptable driving record.
- Ability to successfully complete required background screenings and pre-employment requirements in accordance with applicable regulations.
- Ability to complete required training including medication administration and CPR/First Aid if applicable.
Physical Requirements

- Ability to stand walk bend reach stoop and lift throughout the workday.
- Ability to safely assist individuals with transfers and mobility using approved techniques and adaptive equipment.
- Ability to lift move or assist with up to 100 pounds when required and in accordance with safety procedures.
- Ability to operate a manual wheelchair and other adaptive equipment.
- Ability to respond effectively in emergency situations.
Work Environment
This position is performed in community residential homes and community settings. Employees may work independently or as part of a team and should be comfortable working in a dynamic environment that includes community outings recreational activities and varying levels of physical activity.
