Job Summary
Job Title: Licensed Practical Nurse (LPN) Primary Care
No. of Positions / Candidates Required: 3 FTE (2080 hrs/year each; 6240 total per period)
Specialty: Primary Care Nursing (ICU Med-Surg Telemetry/Cardiac or ER experience desirable)

Location: 4100 W. 3rd St. Dayton OH 45417
Duration: 1-year contract with the possibility of 1 additional year extension
Shift Schedule: 8:00 AM 4:30 PM (0) Mon Fri. Two 15-min breaks 30-min lunch.
How Many Shifts Needed Per Month: Full-time equivalent (2080 hrs/year per LPN)
Credentialing / Onboarding Time: 6 8 weeks.
License: Current Ohio LPN license
Certifications: Current BLS from AHA. ACLS required.
Education: Graduation from ACEN or CCNE-accredited nursing program
Experience Needed: Minimum 1 year direct patient care within the last 3 years. VA experience desirable.
Special Skills Needed: Patient assessment and care planning; emergency drug knowledge; EKG and cardiac monitor operation; EHR documentation; central line and ventilator care per HCS policy; HAPU prevention.
Responsibilities: Same scope as RN above within LPN scope of practice. Includes patient history care documentation medication administration emergency response discharge planning and EHR documentation before end of shift.
