Job Summary

GeneralSummary:Under the supervision of the Site Supervisor the Criminal Justice Liaison is responsible for outreach to members who are involved in the criminal justice system or are currently in jail. The Liaison will coordinate with the Pima County and State courts the member and the treatment Team to ensure timely follow through on community re-entry appointment adherence and additional support during follow up liaison will coordinate and monitor successful compliance with mental and physical health RESPONSIBILITIES:Successfully coordinate with criminal justice intensive outreach and peer support services to members in order to avoid gaps in services and to provide continuity of care (including but not limited to: diversion educational activities med cueing environmental assessments transportation referrals ART meetings etc.).Coordinate with Treatment Team criminal justice resources court system probation/parole officers mental health court restoration of competency services for the Superior psychoeducational groups for members involved in the criminal justice system to decrease documentation for behavioral health record in accordance with agency and funding source in the assessment evaluation and planning of patient case management health promotions skills training and assessment when engaging with members involved in criminal justice community and home-based services to enhance and achieve treatment a minimum number of documented client contact hours per appropriate documentation (charting) and documents accurately and timely in accordance with agency and funding source transportation of members to and from appointments when outreach and follow-up on crisis contacts and missed member advocacy and education as needed to ensure delivery of required other related duties in accordance with agency growth and the human dignity and the confidentiality of each other duties as assigned or necessary as it relates to the general nature of the :Education and Experience Bachelors degree in a behavioral Health related field with one year behavioral health experience OR BHT level with two years behavioral health experience specific to program 21 years of Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).CPR First Aid & AED certification if required (must possess upon hire and maintain throughout employment).Valid Arizona Drivers License 39-month Motor Vehicle Report proof of vehicle registration and liability insurance that meet company insurance requirements if TB test result if required (Employer provides) Questions about this position Contact us
