Essential Functions:
- Provide direct supervision to Care Coordinators and Care Navigators and ensures alignment with The Center for Family Safety and Healing (TCFSH) protocols policies and best practices.
- Ensure timely and effective coordination of services including referrals follow-up and documentation.
- Provide onboarding and ongoing training to staff with an emphasis on trauma-informed care safety planning and resource navigation.
- Support staff in addressing complex client needs including high-risk and crisis situations.
- Track program metrics and outcomes supporting continuous quality improvement efforts.
- Support implementation of organizational initiatives.
- Identify trends gaps and opportunities to improve access and service delivery.
Education Requirement:
- Bachelors Degree in Social Work Counselling Public Health or related field required.
- Masters Degree preferred.
Licensure Requirement:
Licensure (e.g. LSW LISW LPC LPCC) preferred.
Skills:
- Ability to develop and implement an integrated care coordination model across TCFSH programs.
- Ability to establish standardized workflows referral pathways and care management protocols.
- Strong verbal and written communication skills with the ability to clearly convey information to team members healthcare providers and families in a professional and tactful manner.
- Ability to coach team members through change provide constructive feedback and recognition delegate tasks effectively and make informed decisions.
- Ability to foster collaboration and build positive working relationships within the team.
- Ability to effectively interact with individuals from diverse age groups and backgrounds.
- Strong organizational skills with a high level of attention to detail.
- Ability to ensure services are trauma-informed survivor-centered culturally responsive and equitable.
- Ability to promote continuous quality improvement and evidence-informed practices.
- Proficiency in Microsoft Office applications including Outlook Word PowerPoint Excel and Teams.
Experience:
- Three years of experience in care coordination case management or a related human services field required.
- Prior supervisory or team lead experience preferred.
- Demonstrated experience working with individuals impacted by trauma family violence or complex social needs.
- Experience in healthcare child welfare domestic violence or community-based settings.
- Experience with Electronic Health Record (EHR) preferred.
