Work from home eligible up to two days per week after 90 days of : $55000 annual salary including company benefitsJob Summary: The Consultation Coordinator plays a critical role in ensuring the effective and efficient delivery of Community First Services and Support (CFSS) Consultation Services. The Consultation Coordinators primary responsibilities involve personalizing services ensuring compliance and coordinating with stakeholders to deliver exceptional this position you are responsible for coordinating care maintaining service quality managing documentation and advocating for the participants served. Services must be delivered in alignment with regulatory standards and best Job Duties:Individual Service Coordination:Facilitating & Managing Meetings: Schedule attend and facilitate Annual Semi-Annual or Six-Month Progress Review meetings as well as any meetings requested by the participant or their -Centered Planning: Develop implement and monitor individualized service plans tailored to the unique needs and preferences of each Coordination: Collaborate internally with team members and externally with service providers family members and other stakeholders to ensure seamless service Compliance: Ensure all aspects of service delivery align with CFSS regulations and applicable Minnesota state Assurance: Monitor the quality of services to ensure they meet the standards outlined in service agreements and comply with regulatory Provision: Identify and provide access to additional resources or supports that staff and responsible parties may require to fulfill their roles Records: Maintain comprehensive accurate records of service delivery including individualized service plans progress reviews changes in condition and incident : Prepare and submit required reports and documentation to regulatory agencies and other relevant stakeholders as Support: Provide families with support and information about available services ensuring they understand their roles and the supports assessed and approved by county Agreement Monitoring: Oversee budgetary aspects of service provision ensuring expenditures align with the approved service line budget for each Learning & Training: Stay informed about best practices regulatory updates and developments in the field to enhance service delivery. Complete internal training requirements as directed by program supervisors or Orion AssociatesEducation Requirements:Have a bachelors degree or Higher in Social or Human Services FieldsHave at least one year of experience working directly with people who have an intellectual disability or other developmental disability and have an associates degree or higher in a discipline associated with Social or Human ServicesEducational Requirements Substitution:Staff working directly with people can substitute one of the following for a bachelors degree:One year of full-time experience providing direct services to people with disabilities or people older than age coordinating or directing services for people with disabilities or people older than age 65 including self-directed coordinating their own servicesRequired Qualifications:Familiarity with CFSS regulations and Minnesota state lawsAbility to develop and monitor individualized service plansCommitment to person-centered practices and quality assuranceBe age 18 or olderSuccessful clearance of Department of Human Services background checkSuccessful clearance of Motor Vehicle Background Check and acceptable driving record per Company Policy
