This role is based in Rutherfordton NC and combines community-based support with a significant amount of virtual coordination documentation and follow-up.
Work is split between:
- Community settings (home visits local environments public spaces)
- Virtual meetings (care coordination team communication service planning follow-ups)
You will work with individuals who may be in underserved areas or not currently connected to stable services. A large part of the work is maintaining contact and building consistency over time.
- Support individuals experiencing mental health or substance use challenges in community and virtual settings
- Meet clients where they are physically and in terms of engagement level
- Maintain regular contact including when participation is inconsistent
- Coordinate care plans with internal teams and external partners
- Document visits virtual sessions and progress according to agency standards
- Help individuals connect to services resources and supports available locally
- Identify barriers to care access in underserved areas and adjust approach accordingly
- Bachelors degree in human services social work psychology or related field preferred
- Experience working with mental health or substance use populations preferred
- Comfort working independently in community settings and structured virtual environments
- Strong written communication for documentation and coordination
- Valid drivers license and reliable transportation required
- Combination of in-person community work in Rutherfordton NC and scheduled virtual meetings
- Frequent coordination across different levels of service engagement
- Work with individuals who may be entering services for the first time or re-engaging after gaps in care
- Exposure to situations where support systems are limited or developing
- Direct impact on access to care in communities with limited resources
