JOB STATUS: Full-time non-exempt
SCHEDULE: Varies; hours may vary to accommodate needs of person served.
Summary:
Provide direct person-centered support to individuals in their homes and communities. This role focuses on promoting independence assisting with daily living needs and ensuring individuals actively participate in meaningful home and community life while maintaining health safety and well-being. Will provide services in Floyd and Clark Counties.
Key Responsibilities:
Deliver services in accordance with each individuals Individualized Support Plan (ISP)
Assist with development implementation and monitoring of ISP goals and objectives
Support individuals with daily living skills personal care and household management
Provide transportation to employment appointments shopping and community activities
Assist with money management including budgeting and basic financial tracking
Teach and reinforce skills that promote independence in the home and community
Foster self-advocacy informed decision-making and independence
Maintain ongoing communication with team members families and service providers
Schedule and attend medical dental and other appointments as needed
Manage medications including ordering storage and documentation per regulations
Provide crisis intervention and emergency support when necessary
Promote and support positive relationships with family friends and the community
Qualifications & Skills:
Strong communication and interpersonal skills
Ability to demonstrate empathy patience and professionalism
Ability to work independently and manage multiple responsibilities
Understanding of person-centered practices and advocacy
Ability to support individuals with diverse needs and abilities
Strong organizational and documentation skills
Physical & Work Requirements:
Ability to work a flexible schedule including 1st/2nd shifts and weekends
Ability to provide personal care (e.g. toileting bathing feeding) with dignity and respect
Ability to assist with transfers and mobility support
Frequent standing walking bending and lifting
Ability to respond effectively in emergency situations
On-call availability as required (supporting a limited number of clients)
May be required to provide support in other Community Living programs (e.g. Supported Living Respite) as needed
Valid drivers license reliable transportation and proof of insurance
Certifications & Training Requirements:
CPR First Aid and Bloodborne Pathogens certification
Certified Restraint Training
Core A & B certification
Completion of required agency and regulatory trainings (e.g. Human Rights Safety Medication Administration)

Education/Experience Requirements:
