Job Summary
Southwest Center is hiring Community Living Support Professionals to provide person-centered support to adults with intellectual and developmental disabilities in home and community settings. This position focuses on helping individuals build independence participate in their communities and maintain a safe and meaningful daily routine.
Community Living Support Professionals assist individuals with daily tasks community involvement personal growth and skill development while promoting dignity choice and inclusion.

Minimum Qualifications
Applicants must:
Applicants must:
- Be at least 18 years old
- Possess a high school diploma or GED
- Have experience supporting individuals with developmental or intellectual disabilities preferred
- Be able to complete all required agency and regulatory trainings
- Maintain CPR and First Aid certification requirements
- Successfully pass required background screenings
- Pass pre-employment and random drug screenings
- Hold a valid drivers license with acceptable driving history
- Maintain proof of current automobile insurance
- Be dependable and able to work assigned weekends and holidays as needed
Position Responsibilities
Responsibilities may include:

- Providing direct support in both residential and community environments
- Assisting individuals with personal care meal support medication assistance transportation and daily routines
- Supporting individuals in developing independent living and social skills
- Encouraging participation in community activities and personal interests
- Maintaining accurate service documentation within agency systems such as EVV/OASIS
- Following individualized service plans and documenting progress toward goals
- Working cooperatively with guardians family members supervisors and interdisciplinary teams
- Helping maintain a safe respectful and supportive environment
- Attending required meetings trainings and continuing education sessions
- Following agency policies procedures and attendance expectations
- Communicating professionally and contributing positively to the workplace team environment
