Community Resource Specialist

Citizens Memorial Hospital (CMH)is recruiting aCommunity Resource Specialist to work with our amazing team in Bolivar Missouri someone who leads with skill compassion and a drive to positivelyimpactevery patient and family we serve.

Position Details

• Status: Full-time
• WorkShift:Days
• Hours:Monday-Friday 8am-5pm

What Your Day Looks Like:In this role you will support patients navigatingthe healthcare system removing barriers to care and referring to community resources to address social determinants to health for patient safety.

As aCommunity Resource Specialist you will be:

• Providing direct support to patients and families by addressing socio-economic factors impacting health both in-person and by phone.
• Receiving referrals for specialty social service support for clinic patients and providing timely follow-up.
• Performing social needs screening and advising of applicable resources and solutions available.
• Documenting patient-centered plans of care.
• Collaborating and coordinating care with an interdisciplinary care team and advocating for patient safety in complex and challenging situations.

Qualifications

• Education: High school diploma
• Skills: Written and oral communication skills time management detail-oriented
• Licensure/Certification:Willingness to complete CHW certification within 6mo of hire; Preferred: Current CHW Certification
• Physical Requirements:Prolonged periods of sitting to perform computer tasks
