Job Summary

For over 125 years Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children youth adults seniors and those struggling with mental OF THE JOBThe Community Outreach Specialist works in collaboration with staff volunteers and interns in the execution of assigned tasks including but not exclusive of service coordination comprised of intake screening needs assessment information and referral community linkage and outcomes follow up. This work is conducted in a variety of settings: satellite locations and community events throughout Brooklyn & Queens. Responsibilities include but are not limited to: adherence to all administrative and data collection policies collaboration with internal and external networks and engagement in community building activities in support of the agencys mission. This work will be completed within the model of care which is holistic family centered strengths-based and serves to empower clients and enrich the agency has embraced the concept of a fully integrated system of care for all consumers of service. This requires that the staff assess the clients needs holistically and see it as their responsibility to connect the client with services needed--internal or external to the agency regardless of the door to which the consumer enters the system. Travel to various sites throughout Brooklyn and Queens to provide service coordination as direct service coordination to clients including intake documentation needs assessment service/referral planning and linkage and follow-up on referral with the Office of Mission to respond to emergency community eligibility for and facilitate the distribution of available funds from Emergency Food and Shelter Program (EFSP) and other grants/funding sources to financially assist internal and external linkages while engaging in community building activities and promoting program services through outreach timely accurate and thorough documentation of services and referrals in agency databaseTrack emergency funds assistance and utilization in agency in partnership with Call Center staff to provide high quality and timely access to services to all clients entering the in program planning staff meetings case reviews and group in development of on and off-site events affiliated with food pantries parishes and other community event sites and works in collaboration with Parish and Community Relations Directors on such as degree in Social Work Human Services or related field.2 years of experience working with low income families is preferred. Proficient in professional social work generalist practice communication (written & oral) interpersonal and transactional English/Spanish English/Arabic English/Cantonese/Mandarin and/or English/ Russian sitting standing walking & climbing to lift 30 to work flexible hours (evenings/weekends) as to travel throughout the five boroughs as needed.
