Skilled Nursing Sales & Admissions
As a Community Liaison you will serve as the primary connection between our skilled nursing facility and the broader healthcare community. Youll be the first point of contact for referral sources families and discharge plannersguiding tours coordinating admissions and building strong relationships that support census growth and continuity of care.
Youll also play a key role in following patients after discharge ensuring smooth transitions and strengthening long-term partnerships with hospitals physicians and other healthcare providers. This is a highly visible relationship-driven sales role that blends healthcare knowledge communication and community outreach.
The Community Liaison supports skilled nursing census growth through proactive relationship management referral development and admissions coordination. This role works closely with facility leadership to position the community as a trusted post-acute care partner while upholding Americares mission values and Hometown Hospitality standards.
Community Liaison Admissions Liaison Marketing Liaison Healthcare Sales Representative or Referral Coordinator.
- Serve as the initial point of contact for referral sources families and responsible parties
- Coordinate tours and support the admissions process
- Collaborate closely with the Administrator and nursing leadership to support census goals
- Build and maintain strong relationships with hospitals discharge planners physicians and community partners
- Represent the community at networking events promotions and outreach activities
- Present services in a professional engaging and consultative manner
- Assess appropriate placement options for prospective residents
- Conduct follow-up calls and visits after patient discharge
- Maintain a visible professional presence in the community
- Uphold Americares mission values and Hometown Hospitality standards
- LPN RN BSW MSW or therapy background (PT OT ST PTA or COTA) preferred
- Case management admissions or skilled nursing experience preferred
- Minimum 18 years of age with a high school diploma or GED
- Excellent communication and interpersonal skills
- Ability to build trust and long-term professional relationships
- Strong judgment organization and follow-through
- Professional appearance and comfort in a highly visible community-facing role
- Valid drivers license with a good driving record
- Full-time position with up to 50% travel
- Frequent standing walking driving and public interaction
- Ability to lift up to 35 lbs. and push/pull up to 100 lbs.
- Indoor office environment with regular community-based travel
