Mon Wed Thur 8:00am-4:30pm
Tues 8:00am-8:00pm
Fri 8:00am-12:30pm
Evenings and Weekends as required.
Must be fluent in Spanish both written and spoken Language!
A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has anunusually close understanding of the community served. This trusting relationship enables the CHW to serve as aliaison/link/intermediary between health/social services and the community to facilitate access to services and improve thequality and cultural competence of service delivery. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through arange of activities such as outreach community education informal counseling social support and advocacy.
JOB SPECIFICATIONS AND CORE COMPETENCIES
Education: High school diploma or equivalent.
Licensure: Basic Life Support (BLS) certification is preferred.
Experience: One to three years of community involvement experience. Meets additional state/local requirements.
Other Job Requirements:
1. Ability to read and comprehend simple instructions short correspondence and memos. Ability to write simplecorrespondence.
2. Ability to effectively interview clients and accurately record medical information. Ability to effectivelypresent information in one-on-one and small group situations to clients and other employees of the organization.
3. Ability to effectively relate pertinent information with medical and social service professionals.
4. Collecting & Reporting Confidential Information - Understands and is capable of using multiple technologies includingbut not limited to desktop/laptop computers mobile devices and tablets in order to accurately and in real-time capturerelevant social determinant information for the medical record.
5. Must possess a comprehensive knowledge of the local community.
6. Work requires drivers license in good standing the ability to drive a vehicle or have access to reliable transportation.
7. Work requires ability to communicate orally and to hear or communicate with target population. Bi-lingual skills towork with patients in their native language are preferred.
8. Work requires proofreading and checking documents for accuracy.
9. Must be comfortable operating in a collaborative shared leadership environment with clinical supervision.
10. Must possess a personal presence that is characterized by a sense of honesty integrity and caring with the ability toinspire and motivate others to promote the philosophy mission vision goals and values of SJHS.
11. Must be able to set and organize own work priorities and adapt to them as they change frequently. Must be able towork concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having
diverse personalities and work styles.
12. Assigned hours within your shift starting time or days of work are subject to change based on departmental and/ororganizational needs.
