Job Summary
The OpportunityThe Community Health Worker II is responsible for providing advanced health education outreach and care coordination services to individuals and families in the community. Areas could include exercise mental health nutrition health care system navigation substance use and other health behaviors. You will also closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes.
Essential Functions
-Conduct in-depth assessments of patient needs goals and barriers to achieving good health outcomes
-Provide advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions
-Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs
-Assist patients in navigating the healthcare system and accessing appropriate services including health insurance and social services
-Provide ongoing follow-up and support to patients to ensure continuity of care and successful achievement of health goals
-Develop and implement health promotion programs and activities to address community health needs
-Collect data and maintain accurate records of patient interactions and outcomes
-Attend meetings and trainings related to community health promotion and education
Qualifications
Qualifications:
Bachelors Degree Public Health Social Work or other related field of study required. Related experience can be accepted in lieu of degree.
2 years of experience in community health outreach health education or related field required.
Experience with community-based work patient home visits SUD homelessness and a complex patient population required.
Experience with Social Determinants of Health (SDOH) housing food insecurity transportation utilities employment childcare assistance programs required.
Experience helping patients or clients apply for state/federal programs/benefits (i.e. SSDI SSI SNAP) and advocating for the patient with appeals required.
Knowledge of / familiarity with transportation resources (PT1) required.
Community Health Worker certification strongly preferred

Demonstrated ability to work effectively and provide advocacy for all populations and communities.
Strong communication and interpersonal skills with the ability to interact effectively with various populations.
Ability to work independently and as part of a team.
Basic computer skills including Microsoft Office and database management.
Bilingual skills preferred.

Schedule and Work Model:
Full time Monday through Friday standard business hours.
Hybrid: Approximately 3-4 days/week in the community (including patient home visits) in the Cambridge MA/Medford MA area (as far as Burlington MA / Lexington MA) and approximately 1-2 days/week remote / work from home.
Remote workdays require stable quiet secure compliant workstation using MGB issued equipment.
