Job Summary
A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has aremarkableunderstanding of the community. This trusting relationshipwith the communityenables the CHW to serve as a liaison/link/intermediary between healthsystem and the community to facilitate access to services and improve the quality and culturalproficiencyof service delivery.In this role works to improve health outcomes and eliminate racial ethnic and economic disparities in those outcomes.Plays an important role inbuildingindividual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreachhealtheducation informal counseling social supportand advocacy.
Requirements
1. Must possess a comprehensive knowledge of the local communitybased on personal lived experienceand the ability to articulate the lived experience and perspective.
2. High school diploma or GED.One to three(1-3)yearsofcommunity involvementexperience.MustobtainaTrinity Healthor equivalentCHWcertificationwithinthreemonths of hire.
3. Ability to read and comprehend simple instructions short correspondence and memos. Ability to write simple correspondence.
4. Work requires ability to communicate orally and tohear orcommunicate withfocuspopulation.Bi-lingual skills to work with patients in their native language are preferred; certification as aqualifiedmedical interpreter/translator is a plus
5. Basic Life Support (BLS) certification is preferred.
6. Must be comfortableworking and serving in a diverse and inclusive environmentandoperating in a collaborative shared leadership environment with clinical supervisionwhere applicable.
7. Must possess a personal presence that is characterized by a sense of honesty integrity and caring with the ability to inspire and motivate others to promote the philosophy mission vision goals and values of Trinity Health.
Pay Range
$23.00 - $28.65
