The Community Health Assistant will generally support the administrative and coordination tasks of the department which will include but not be limited to responding to emails and calls from community members processing & tracking health fair and sponsorships requests outreach to internal departments enrolling community members in education classes and event and class preparations
Community Outreach Event and Sponsorship Tracking Assistance
Assist with monitoring the Community Health email processing community requests via the smartsheet and reporting requests to the Director of community health.
Assist with outreach to departments to secure their attendance at community health events if appropriate.
Assist with sending calendar invitations and confirmation for events.
Assist with material and supply preparations as needed.
Assist with special events as needed.
Diabetes Prevention Program
Send emails to program participants regarding upcoming class sessions including distribution of participant materials
Organize participant documentation into participant folders for accurate tracking and reporting.
Enter the participant data into the appropriate google tracking sheets.
Assist participants with securing class recordings as requested.
Escalating concerns to the appropriate staff member when needed.
Living Well Workshops- Diabetes Self-Management Classes
Track enrollment for the Living Well Diabetes Self-Management (DSM) classes; calling and emailing participants to complete enrollment.
Send weekly reminder emails to registered participants.
Assist with outreach and recruitment efforts for the classes.
Assist with on-site class logistics as needed.
Assist with distributing evaluations as needed and data entry of results.
Requirements
Required Qualifications:
Education: Bachelors degree required
Experience:
1-3 years of administrative and coordination experience
Experience with interacting with people in the community
Excellent communication skills
Strong organizational skills
Adaptability and flexibility
Initiative and self-motivation
Teamwork and collaboration
Administrative and technology skills
Reliability and accountability
