The pay for this position is $44850-$58909 annually.

$500 sign-on bonus

The Community Case Manager works directly with individuals receiving services to build functional skills address barriers to care and social and mental wellbeing and support follow-through on treatment plans and referrals. Our ideal candidate will have strong collaboration problem-solving and interpersonal skills.

• An Associates degree and two years experience and/or a bachelors degree
• Knowledge on how to provide holistic and therapeutic case management services.
• Experience working in the mental health field.
• Computer and documentation skills.
• Ability to solve problems and deal with complex situations and stay calm.
